Community Based Organizations

Helping Community Based Organizations Establish Successful Healthcare Partnerships

Community Based Organizations (CBOs) have a key role in improving the healthcare delivery system. CBOs and healthcare providers must partner effectively to advance health equity and drive successful and sustainable system transformation.

澳门六合彩 and 澳门六合彩 Community Strategies work with CBOs across the country to build healthy communities. We identify areas where healthcare and social needs intersect and create connections between healthcare and community organizations. 澳门六合彩 understands the unique dynamics of CBO engagement and what it takes to create successful healthcare partnerships.

Our experts are ready to help with innovative, real-world solutions for everyone in the CBO sphere, from delivery systems promoting CBO engagement to CBOs forming collectives or contracting independently to promote population health.

澳门六合彩鈥檚 personalized CBO Engagement Toolkit helps CBOs, Managed Care Organizations (MCOs) and Healthcare Organizations (HCOs) evaluate resources and identify opportunities to partner.

The Toolkit鈥檚 tailored resources include the Capacity Assessment Tool and CBO Engagement Training Series. Both are client-specific and personally delivered to give you the exact resources and training to meet your needs.
